Video, telephone or in person?

Many problems can be dealt with well by video or telephone. Others need the doctor to listen to your chest, look in your ears or examine you, and those are best seen in person. The full lists below mark which is which.

Can be done by video or telephone

The doctor can work from your history, plus things you send or read out to us, such as:

  • Your history alone, for example colds, sore throat, hay fever, menopause and HRT review, contraception, mental health review and medication review
  • A clear photograph, for example eczema, psoriasis, conjunctivitis, fungal skin infections, impetigo, shingles, chickenpox, scabies, insect bites and stye (never of an intimate area)
  • Results you already have, for example blood test results and blood pressure readings you take at home
  • A validated symptom score and history, for uncomplicated cystitis in women

By video or telephone we can review, advise, and prescribe where it is safe and within our prescribing rules.

Best seen in person

The doctor needs to look, listen or examine, for example:

  • Chest infections, asthma review, COPD and bronchiectasis review
  • Ear infections (the doctor uses an otoscope)
  • Flu and COVID-19, glandular fever, vomiting and diarrhoea
  • Mild skin infections such as cellulitis, and ingrowing toenails
  • Back and shoulder pain, tension headache, hernias
  • Diverticular disease, recurrent cystitis review, vertigo other than BPPV
  • General health checks
  • Mild croup, colic, and constipation in children

In person means a mobile doctor visit at your home or at a temporary address such as a hotel.

If the doctor decides during a video or telephone consultation that you need to be examined, we will arrange an in-person visit where we can, or direct you to your own GP, NHS 111 or emergency services.

Emergencies and urgent problems

  • Vomiting blood, coughing up blood, rectal bleeding
  • Head injury, fractures, burns, bites and wounds
  • Anything likely to need hospital admission

Problems that need an examination we cannot do

  • Breast symptoms and breast examination
  • Pelvic, genital, rectal and prostate problems

  • Thrush and bacterial vaginosis

  • Fit notes, driving medicals (DVLA) and smoking cessation
  • Palliative and end of life care
  • Controlled drugs (other than testosterone in menopause care)

We also cannot consult by video or telephone about anything the doctor decides needs a physical examination. In that case we will arrange an in-person visit where we can, or direct you to your own GP, NHS 111 or emergency services.

  • Premature ovarian insufficiency
  • Hot flushes, mood symptoms and vaginal or urinary symptoms of menopause
  • Menopausal symptoms in women who have had breast cancer: non-hormonal treatment, and local vaginal oestrogen where agreed, only with the documented agreement of your oncology or breast team
  • Testosterone for low sexual desire linked to menopause, where HRT alone has not helped. This is an off-label use, in line with NICE NG23 and British Menopause Society guidance
  • Premenstrual syndrome (PMS)
  • Premenstrual dysphoric disorder (PMDD), not severe and not in crisis
  • Combined and progestogen-only oral contraception: assessment and prescribing, with a risk assessment By video or telephone: uses blood pressure readings you take yourself, plus your personal and family history
  • Oral emergency contraception, with advice on the time window. We refer to a sexual health service where a copper coil would be more effective, where the window has passed, or for STI screening where appropriate
  • Pre-conception advice, including folic acid and lifestyle
  • Delaying a period with tablets, for example for travel or an event
  • Heavy or irregular periods: assessment and first-line management by history only, without examination. Bleeding after sex and bleeding after the menopause are not included. You are signposted for examination where appropriate
  • Bleeding on HRT that fits the expected pattern of breakthrough bleeding, assessed by history only. If the pattern is not reassuring or an examination is needed, you are referred urgently to your GP or a gynaecology service
  • Polyendocrine metabolic ovarian syndrome (PMOS, formerly PCOS), already diagnosed: discussion of symptoms only

Full list of conditions we will not treat

We do not assess or treat the following in any circumstance, whatever the age or gender of the patient, and whether we see you by video, telephone or in person. Where we cannot treat you, we will explain why and help you find the right care. We will not leave you without a clear next step.

Emergencies and urgent problems

  • Any medical emergency or red-flag presentation, including suspected sepsis or meningitis, heart attack, stroke or TIA, new weakness or loss of function, blood clots in the leg or lung (DVT and PE), anaphylaxis, testicular torsion and suspected non-accidental injury
  • Any acute surgical emergency
  • Any problem where you are likely to need admission to hospital
  • Chest pain of any cause
  • Loss of consciousness, collapse or sudden memory loss
  • Delirium and sudden confusion
  • Head injury
  • Burns and scalds
  • Animal and human bites
  • Facial or orbital cellulitis (infection around the eyes or nose). This needs emergency hospital care
  • Septic arthritis
  • Low blood sugar (below 4 mmol/L), diabetic ketoacidosis and hyperosmolar hyperglycaemic state
  • Acute gynaecological emergencies, including ectopic pregnancy

Heart, lungs and circulation

  • Heart attack
  • Angina and heart disease with symptoms
  • Heart rhythm problems and palpitations
  • Heart failure
  • Acute shortness of breath
  • Collapsed lung (pneumothorax) and fluid around the lung (pleural effusion)
  • Aortic aneurysm
  • Peripheral arterial disease, leg pain on walking (claudication), and leg ulcers from vein or artery disease
  • Critical limb ischaemia, gangrene and any other vascular emergency
  • Sleep apnoea

Stomach, bowel and liver

  • Vomiting blood
  • Coughing up blood
  • Bleeding from the back passage and black, tarry stools
  • Faecal incontinence
  • Ongoing or unexplained change in bowel habit (short-lived infectious diarrhoea and vomiting is covered in the list of what we treat)
  • Haemorrhoids, because we cannot examine you to confirm the cause or rule out something more serious
  • Inflammatory bowel disease (Crohn's disease and ulcerative colitis)
  • Sudden abdominal pain or a suspected acute abdomen
  • Acute diverticulitis, suspected or known, including a flare of diverticular disease with pain, fever or feeling unwell
  • Appendicitis
  • A flare of gallstones (cholecystitis or biliary colic)
  • Pancreatitis
  • Groin hernias (inguinal and femoral)
  • Anal fissures
  • Cirrhosis and other chronic liver disease needing specialist care
  • Ongoing management of hepatitis B or hepatitis C

Kidneys, bladder and men's health

  • Kidney infection (pyelonephritis)
  • Kidney stones and renal colic
  • Sudden inability to pass urine
  • Acute kidney injury
  • Chronic kidney disease with eGFR of 30 or below, kidney failure and dialysis
  • Nephrotic syndrome, nephritic syndrome and polycystic kidney disease
  • Urinary symptoms in men, prostate problems and any male urinary problem, because an examination may be needed
  • Stress urinary incontinence, because assessment needs a pelvic examination

Women's health, pregnancy and intimate examination

  • Any intimate examination, including breast, pelvic or vaginal, genital (including testicular), and rectal or prostate examination. You are signposted to your registered NHS GP or another suitable provider
  • Breast symptoms, including a lump, pain, nipple discharge, mastitis or skin change
  • Conditions of the external genitalia, including lichen sclerosus and vulval varicose veins
  • Thrush and bacterial vaginosis
  • Sexually transmitted infections, including genital herpes
  • Any photograph of an intimate or genital area, for any purpose
  • Bleeding after sex
  • Bleeding after the menopause that is not related to HRT, or that does not fit the expected pattern on HRT. You are directed to arrange an urgent specialist referral or an urgent GP review
  • Any pelvic mass
  • Pelvic organ prolapse and rectal prolapse
  • Pregnancy, labour, birth, antenatal and postnatal care, and all maternity care
  • Cervical screening
  • Fitting or removing a coil or contraceptive implant
  • Fertility treatment and IVF

Cancer, lumps and skin

  • Any new lump or swelling that needs a physical examination, including swollen glands or a neck lump, because of the possibility of cancer
  • Cancer care of any kind, including any consultation where cancer is suspected or where you want reassurance that cancer has been ruled out (the only exception is treating menopausal symptoms in women who have had breast cancer, agreed with their oncology or breast team)
  • Skin lesions that could be cancer
  • Shingles affecting the eye
  • Minor surgery, stitches, wound care and dressings, draining abscesses, joint or soft tissue injections, and removing or biopsying skin lesions
  • Surgical procedures of any kind

Eyes, ears, nose, head and nerves

  • A new, undiagnosed change in eyesight, eye pain with sensitivity to light, or a red eye with disturbed vision
  • Tinnitus and sudden hearing loss
  • Ear injury
  • Nosebleeds, because treatment may need cautery or packing
  • Sudden low back pain, or any back pain with red-flag signs, including suspected cauda equina syndrome
  • Sudden or thunderclap headache, or headache with red-flag signs such as fever, neck stiffness, new weakness or head injury
  • Giant cell arteritis (temporal arteritis)
  • New facial weakness or suspected Bell's palsy
  • Any muscle or bone problem caused by injury, or a suspected fracture or dislocation
  • Parkinson's disease
  • Epilepsy and seizures of any kind, including review of an existing epilepsy diagnosis
  • Brain or nerve conditions under a neurologist, such as multiple sclerosis, brain tumour, the after-effects of stroke or hydrocephalus
  • Assessment of dementia and memory problems

Mental health and behaviour

  • Thoughts of suicide, self-harm, and drug or medication overdose
  • Mental health crisis, and any problem needing urgent psychiatric help
  • Severe depression, severe anxiety disorders and severe OCD
  • Schizophrenia and other psychotic disorders, mania in bipolar disorder, and personality disorder needing specialist input
  • Any mental health problem in a child or young person under 18
  • Eating disorders
  • Substance misuse
  • Alcohol dependence and alcohol withdrawal
  • ADHD: diagnosis, management or shared care
  • Assessment for autism
  • Learning disabilities and learning difficulties, including annual health checks and any other review
  • Smoking cessation support

Children and young people

  • Bedwetting (enuresis)
  • Feeding concerns in babies and children
  • Faltering growth and failure to thrive
  • Growth and development concerns, including early or delayed puberty
  • Any injury in a child, including head injury
  • A child who is unwell or feverish with red-flag signs (using the NICE traffic light system for children under 5)
  • Genetic and congenital conditions needing specialist diagnosis or care
  • Scheduled child health checks, such as the 6 to 8 week check and developmental milestone checks
  • Nappy rash, and any problem in the nappy area that needs a look at intimate skin
  • Missed or absent periods in girls under 18 (secondary amenorrhoea)

Long-term and specialist conditions

  • Ongoing monitoring and management of any long-term condition, including one we diagnose. We inform your registered GP and refer you back for ongoing care (menopause care is the exception)
  • Acting as your main doctor for the long-term management of chronic disease. This stays with your registered GP or specialist
  • Addison's disease
  • Prolactinoma
  • Cushing's syndrome and other pituitary, adrenal or parathyroid disorders
  • Lyme disease
  • Malaria and other tropical or notifiable infectious diseases
  • Tuberculosis
  • Sickle cell disease, including sickle cell crisis
  • HIV medicine
  • Frailty and comprehensive assessment of older people
  • Falls risk assessment

Services and prescribing we do not provide

  • Vaccinations and immunisations, including travel vaccinations
  • Travel health advice
  • Fit notes (Statement of Fitness for Work)
  • Fitness to drive and DVLA medical assessments
  • Palliative care and end of life care
  • Completing or signing Do Not Attempt Cardiopulmonary Resuscitation (DNACPR) forms
  • Prescribing controlled drugs, other than testosterone for menopause as listed above
  • Prescribing GLP-1 medicines or any other medicine for weight loss

We also do not consult by video or telephone about anything the doctor decides needs a physical examination to reach a safe diagnosis or plan. If this becomes clear during a consultation, we will arrange an in-person visit where we can, or direct you to your own GP, NHS 111 or emergency services. Photographs of skin conditions are not treated as needing a physical examination for this purpose (see the list of what we treat).
