Oestrogen and serotonin

Oestrogen regulates the serotonin system. As oestrogen declines in perimenopause, serotonin availability falls. A direct biological pathway from hormonal change to anxiety and low mood.

Progesterone and GABA

Progesterone metabolises into allopregnanolone, a natural calming agent acting on GABA receptors. Falling progesterone means a nervous system more easily activated and harder to calm.

Cortisol and the HPA axis

Hormonal fluctuation activates the stress response. When oestrogen is unstable, cortisol is more easily triggered and harder to bring down. Wired but exhausted. This is the mechanism.

The misdiagnosis problem

Women with new-onset anxiety in their forties are frequently prescribed SSRIs without a hormonal assessment. The treatment may be incomplete. A full picture requires a full assessment.
